Name(s), or name of clinic or business 2. Address 3. Phone number, website url 4. Description of services in 40 words or less- please also include your email address for us so we can notify you of your listing approval, and communicate with you through email Step 2- After your information has been reviewed and approved (you will get an email) you can set up payment with paypal by clicking the link below. Paypal will take $10.00 usd out of your account once a month.
